Keloid or piercing bump? How to tell what that lump is
Most pages answering this question are written by jewellery shops. Here is what the research actually supports. It includes one popular rule we could not trace to any study — so we will not repeat it.
The one line that matters
A keloid grows beyond the edge of the original wound. In a pierced ear, that means beyond the piercing hole itself. A lump that stays at the piercing site is, by definition, something else. Under the microscope this shows as a "tongue-like advancing edge" pushing under normal skin. That edge is found only in keloids.

What else a piercing lump can be
"Minor infections, allergic contact dermatitis, keloid formation, and traumatic tearing may occur after piercing of the earlobe. 'High' ear piercing through the ear cartilage is associated with more serious infections and disfigurement."

A pyogenic granuloma is a harmless overgrowth of tiny blood vessels. Skin can produce one after minor injury or ongoing rubbing — piercings included. It looks alarming: red-purple, and it bleeds very easily at a touch. But it can settle on its own, and on the ear it is rare. It is not a keloid and does not behave like one.

The timing rule we will not print
Almost every piercing-care page repeats a version of "bumps appear within weeks, keloids take months". We searched for a study behind that rule. We could not find one. So it is not on this page. If a traced source turns up, we will add it — with the citation.
Why even doctors find the edge cases hard
Even under a microscope, keloid and hypertrophic scar are sometimes hard to tell apart. The tissue feature counted as the hallmark of keloid showed up in only about half of the keloid samples in the study that compared them.

If it is a keloid, where it sits on the ear matters enormously
Same study, same treatment, same follow-up — and a seven-fold difference depending on which part of the ear the keloid grew on:
- 5.7% Earlobe (lobule) 2 of 35 keloids came back everyone watched for at least 18 months
- 38.5% Auricle excluding earlobe (helix / concha) 5 of 13 keloids came back everyone watched for at least 18 months
